Meal Benefit Forms Due by 9/30

Please complete and submit all meal benefit forms by 9/30. Verification is required for processing so make sure you submit one of the following with your form: pay stub, W-2, social security or any other document with listed income. If you do not submit by the 30th, your child will be changed to a paid account and held accountable for payment. Any charges made prior to submission of the form must be paid, as qualifying is not retroactive and approval can not be back dated.
